3 Creating Your Dashboard Account STAFF USER ACCOUNTS FOR SENDING INVITES

4 Staff members who will create patient portal accounts have been sent registration emails to create their own Follow My Health dashboard account. Clicking on the registration link will guide you through the steps.

5 Microsoft Silverlight During the dashboard creation process, you may receive a warning stating that “Silverlight” must be installed before proceeding. This is a free Microsoft web application that is needed to properly run the dashboard in your browser. Please click to download and run this program. If you receive an error message stating that you do not have the permissions to do so, please contact Lynn Hornby.

6 Click the FMH icon to begin account registration Clicking the link within the email invite will take you to the registration screen.

7 Choose your own username and password, and enter your socpa.com email address. Please make note of your login credentials as Administration will not have access. While there is password recovery in place, a forgotten username cannot be recovered.

8 Enter your Security Code and click “I Accept” to agree to the Terms of Service.

10 Types of Patient Portal Accounts  Minor Proxy  Young Adult (Adolescent) Proxy  Adult Account  Adult Proxy

11 Minor Proxy The Minor Proxy is the parent or guardian who communicates on behalf of the child who is newborn through 11 years of age. The proxy will have full access to the patient account, with the exception of Protected Health Information as outlined in the California Minor Consent Laws.

12 Young Adult Proxy (Adolescent) The Young Adult Proxy is the parent or guardian who communicates on behalf of the child who is 12 through 17 years of age. They may request appointments and send messages on behalf of the child, but any new clinical information will not populate into the Young Adult Proxy account.

13 Adult Account and Adult Proxy At 18 years of age, the Young Adult Proxy is automatically terminated, and the patient will need to create their own account to gain access to the portal. At this point, the adult patient may grant and remove proxy access at their discretion within their own portal dashboard.

14 Inviting a Proxy Click on the Patient Registration tab. Patients under 18 will not have their own account, and will be accessed by a “Proxy” (parent or guardian). To invite the proxy, click on “Proxy” for patients newborn through 17 years of age.

15 Search for Patient Enter patient search information, and then click on Search. Make certain the date of birth matches the date listed on the Portal Access Form.

16 Add Patient(s) to Invite Choose the radio button to select patient, and click “Add Patient to Invite”. (You may search for and add additional siblings to a single invite).

17 Click “Invite Proxy”

18 Proxy Alert If you receive a proxy alert, acknowledge by clicking “OK”. Proxy access levels are pre- determined by the patient’s age and you will not need to override this setting.

19 Invite the Proxy Complete the proxy invite using the information provided on the Portal Access Form that was completed by the parent. You will also enter the patient’s 4-digit year of birth as the Security Code. (If signing up multiple family members, choose one birth year as the code for the family). Click your keyboard’s “tab” key and then Click “OK”.

20 Provide parent with portal business card, recording patient name(s) and security/invite code on back.

21 Email Invite Sent! The proxy will receive an email invite with complete instructions, usually within minutes.

22 Sending Portal Invites ADULT PATIENT ACCOUNTS

23 Inviting an Adult Patient Click on the “Patient Registration” tab. Patients over 18 will have their own account, and the invite is sent through the “Invite Patient” tab. Choose correct patient and click “Send”

24 Enter patient information and click “Send”

25 Releases acknowledged by parent during account set up are filed in patient’s chart.

28 Messaging Capabilities  Providers can always initiate web messages from the EHR, and patients/proxies have the opportunity to reply.  If a patient or proxy initiates a web message and a staff member or provider responds, the patient/proxy has the opportunity to reply back.  If a staff member (non-provider) initiates a web message, the patient/proxy can view the message, but is unable to reply back.
