2 Introduction:  Facing an increase in retiring Baby Boomers, CMS is trying to reduce spending on its chronically ill Medicare beneficiaries, who currently generate most of the programs expenses. Most in healthcare know that change is the necessary to continue providing optimal care to patients.  More that two-thirds of Medicare patients have multiple chronic conditions, these account for 93% of Medicare spending. Patients with six or more chronic conditions are more than half of the Medicare costs. Chronic “frequent hospital flyer” patients are who Home Doctor Services have developed this unique program for. Home Doctor Services objective is to partner with ACO’s to help optimize patient care while reducing hospitalizations.  Primary care physicians deliver the bulk of chronic care. With the baby boom population, doctors find they do not have the time to deliver all the recommended chronic care to patients during an office visit. Physicians in the fee-for-service Medicare program are being paid for face-to face visits, not for comprehensive services that are required to adequately manage people with multiple chronic diseases.  Home Doctor Services addresses these chronic care issues at a much lower cost to Medicare without disrupting the primary care physicians and their office staff. Home Doctor Services mission is to be a valued partner to all Primary Care Physicians it serves. Home Doctor Services becomes an extension of the Primary Care Physicians practice.

3  Doctor Home Services Model: The implementation of home-based primary care into a comprehensive patient-centered medical management system that reduces the cost of care for the aging chronically ill; transitioning Medicare’s fee-for-service payment system to an ACO shared-savings basis; aligning the interest of patients, providers and Medicare.  Focus: Home Doctor Services deliver Primary Care At Home for the most complex patient with multiple chronic diagnosis, thus saving costs from “frequent flyer” hospital patients. We shift the focus from higher cost institutions to lower cost care at home through the implementation of the Home Doctor Services program.  Revenue Model: Home Doctor Services generate revenue through fee-for-service and shared savings contracts with Medicare and Accountable Care Organizations to deliver healthcare reform in a commercially sustainable delivery model.

4  Home Doctor Services Visits: By providing comprehensive patient assessments and delivering Primary Care At Home, patients are more likely to remain compliant with a doctor’s prescribed treatment plan, Patients’ office-based PCP’s help the Home Doctor Team become the center of care management from sharing patient information and providing professional oversight. Home Doctor teams work in conjunction with all PCP’s to provide optimal care an continuity of care, while keeping the patients out of the hospital.  Doctor Call Center Access and Remote Patient Monitoring: 24 Hour patient access to a medical decision maker and remote monitoring by providers of chronically ill patients proactively manages and delivers responsive care, thus avoiding the emergency room visits and hospitalizations.  Benefits: There is a current shortage of Primary Care Physicians’ which will only get worse in the future.
