1 ACCEPTABLE SUPPORTING DOCUMENTS For Nutrition Information and Crediting

2 Documents for Nutrition Information 1. Does the product have a Nutrition Facts Panel on package? Yes – you have acceptable documentation. No – go to question 2

3 2. Is the Product a Commodity food not sent to 2 nd party for processing? Yes – can use USDA Food Fact Sheets found at http://www.fns.usda.gov/fdd/nslp-usda-foods-fact-sheets NO – go to question 3 http://www.fns.usda.gov/fdd/nslp-usda-foods-fact-sheets

4 3. Is product a fresh fruit or vegetable? Yes – USDA National Nutrient Database Standards Reference sheets are available at http://ndb.nal.usda.gov/http://ndb.nal.usda.gov/ NO – go to question 4

5 4. Is your product a processed food without nutrition fact label? Yes – contact manufacturer for a specification sheet that is signed, dated and on company letterhead. No - if your product does not fit any above categories – contact KDE for guidance.

6 Documents for Crediting 1. Does the products have a CN Label on package? Yes – you have acceptable document. No – go to question 2

8 3. Is your product a fresh fruit or vegetable or meat/meat alternate without a CN label? Yes – go to question 4 No – go to question 5

9 4. Can you find the exact item in the FBG? FBG can be found at http://www.fns.usda.gov/tn/foodbuying-guide-chile- nutrition-programs http://www.fns.usda.gov/tn/foodbuying-guide-chile- nutrition-programs Yes – Use the FBG to calculate how the product credits. No – go to question 6

10 5. Is your product a grain product with a nutrition label? Yes – use the nutrition label and Exhibit A to calculate how credits. Yes –but package says that it credits for more than Exhibit A calculation / more than it weighs – Contact the manufacturer and aquire a specification sheet that is signed, dated, and on company letterhead explaining how the product credits for more than Exhibit A No – go to question 6

12 6. Is your product a processed food, without CN label, and not available in FBG? Yes – Contact the manufacturer and aquire a specification sheet that is signed, dated, and on company letterhead. No – If your product does not fit into any of the above categories, contact KDE for guidance.
