1. How to Search Records Go to search Tab You can search the records with baby or mother record in Search Text under Search Button; make sure that all other fields are empty and status of forms is selected ---ALL— use other option to search your records

2. Search Text by Institution name NEP-KH-TUTH Or baby or mother records number Search Form Type NewBorn (All) -- NB in Born Only Birth Defect (All BD only) Etc…

3. Select the date range of the form filled Search Form Type NewBorn (All) -- NB in Born Only Birth Defect (All BD only) Etc… Search Text by Institution name NEP-KH-TUTH Or baby or mother records number

4. Select the form Status Select the date range of the form filled Search Form Type NewBorn (All) -- NB in Born Only Birth Defect (All BD only) Etc… And finally click GO button Search Text by Institution name NEP-KH-TUTH Or baby or mother records number
