2 Policy Expeditionary screening is required for all Individual Augmentees (IAs) (GSA, IAMM, and RC Mobilization). This screening is comprised of two separate parts, 1.Medical/Dental 2.Administrative.

3 Medical/Dental Pre-Deployment Requirements The Medical/Dental screening portion requires the completion of the following: Pre-Deployment Health Assessment (PDHA) (DD Form 2795) survey and screening appointment with Provider (within 60 days prior to deployment). The form can be completed by going to the following site: www-nehc.med.navy.mil and following the directions provided in the Expeditionary Screening forms section in this site about completing your PDHA. NAVMED 1300/4 (3-09), Expeditionary Medical and Dental Screening for Individual Augmentee (IA) and DD Form 2807. Any required Area of Responsibility (AOR) theater-specific medical screening forms NAVMED 1300/5 through NAVMED 1300/11.

4 Medical/Dental Pre-Deployment Requirements PHA (within 6 months) Medical Readiness Reporting System (MRRS) updated Dental (within 90 days) (Class 1 or Class 2 designation) – Printed Panoramic and bitewing views HIV (within 120 days – Negative result) PPD Questionnaire (within 6 months) * If (+)PPD Convertor, completion of INH Therapy Immunizations (updated/theater specific requirements) Audiogram (within 1yr) & Baseline Audiogram Optometry/Eye exam (within 2 yrs) **within 1yr if you wear glasses (order gas mask inserts)

5 Medical/Dental Pre-Deployment Requirements PRIMS printout/Documentation of PRT waivers or failures FEMALES * PAP ( within 6 to 9 months) – Benign findings * Mammogram (if > 40 yrs old - within 1 yr.) – Benign findings * Pregnancy test (within 30 days) If documented Boots on the Ground deployment > 30 days, must have a PDHRA completed/certified in the NEHC EDHA database Any Medical/Mental Health/Psych Issues (i.e. High blood pressure, High cholesterol/triglycerides, Orthopedic injuries, Back pain, Depression, Anxiety) must have documentation of STABLE STATUS & TREATMENT with appropriate follow up care as advised.

6 Medical/Dental Pre-Deployment Requirements Medications-must be on stable dose for minimum of 180 days * Must take minimum of 180 day supply * Aspirin—not allowed unless prescribed/documented as medically necessary Follow up of abnormal lab studies, x-rays, and other test PRT/PFA waivers must be in record w/ documentation of that medical issue resolved/stable PRT/PFA Failures—NONDEPLOYABLE(NPQ) if 2 failures in past 3 yrs, unless most recent PASSED

7 Medical/Dental Pre-Deployment Requirements Consults & Referrals to Specialists as well as Therapies such as Physical Therapy, Occupational Therapy & Counseling must be documented as signed off and/or stable without any restrictions; fit for full duty Baseline Automated Neuropsychological Assessment Metrics (ANAM) testing *ANAM - a computerized battery that measures cognitive performance related to attention. memory, and processing of information Medical Warning Tags (if applicable) ******FAILURE OF SERVICE MEMBER TO DISCLOSE ANY & ALL KNOWN MEDICAL CONDITIONS MAY RESULT IN DISCIPLINARY OR ADMINISTRATIVE ACTION!!!!!!!!!

8 Administrative Pre-Deployment Requirements The Administrative portion requires the completion of the following: NAVPERS 1300/21(Revised 07-09), Medical Suitability Certification.
