2 Implementation Implementation is a category of nursing behavior in which the actions necessary for accomplishing the health care plan are initiated and completed.

3 Nursing Interventions A nursing intervention is any treatment, based upon clinical judgment and knowledge that a nurse performs to enhance client outcomes. Interventions can be both direct and indirect care.

4 Direct vs Indirect Care Direct care= treatments performed through interactions with the client. Examples= med.admin., IV placement Indirect care= treatments performed away from the client but on behalf of the client or group of clients. Examples= safety, infection control

5 Types of Interventions Nurse initiated Physician initiated Collaborative Can also be organized, developed, and communicated via: Protocols Standing orders

6 Protocols A written plan specifying the procedures to be followed during the care of a client with a particular clinical condition or situtation. Examples: Admission and discharge protocols K+ protocol in acute care CPR protocol

7 Standing orders A document containing orders for the conduct of routine therapies, monitoring guidelines or diagnostic procedures. It is used with specific clients with identified clinical problems, must be approved and signed by a physician. Examples: Critical care s.o. for giving meds for irregular heart rhythms, standing orders for patients under the care of hospice.

8 Choosing Interventions Desired or expected client outcomes Characteristics of the nursing diagnosis Evidence base for the intervention Feasibility for performing and intervention Acceptability to the client Capability of the nurse

9 Implementation Process Reassessing the client Reviewing and modifying the existing nursing care plan Identifying areas of assistance Implementation of nursing strategies to achieve the goals of care Communicating nursing strategies

10 Implementation Methods Assisting with ADLs Counseling Teaching Giving care to achieve therapeutic goals Giving care to facilitate attainment of health goals Supervising and evaluating personnel
