Slide 2History January 6, 2014 – Revised DD Crisis Response System Plan for children and adults released – Lead CSB overseeing REACH provided brief environmental scan regarding expanding services to youth 14-18. February 4, 2014 – Request for proposals sent to Executive Directors, Regional Project Managers, ID Directors, and REACH Program Directors March 14, 2014 – Regional plans due May/June 2014 – Funds disbursed to Regions: $225,000

Slide 4 Initial Plans Region Specific Proposals Independent Reviewer Recommendations – Assessment of need – Serving all children and adolescents up to the age of 18 who have ID/D and experience a crisis – The continuum of services to include mobile crisis teams, in- home support, respite and access to crisis stabilization and hospitalization – Core staffing – Core training for team members, case managers, providers and CSBs – Outreach to families – The data elements to be tracked to determine if outcomes are being achieved – Program evaluation
