Youth Transition scenario 17 year old medically complex youth with chronic health issues Has Medi-cal insurance Dependent on others for care Frequent hospital admissions Current care includes his primary physician and 5 specialty clinicians Attends school

Ideal planning age Because of the medical complexities of this child an ideal age would be between 14-16 years old

Challenges in finding adult care Medical insurance not accepted Diagnosis is too complex Physician not accepting any more patients Recommended physician is too far away

Success in finding adult care Asking your child’s Primary physicians and/or specialty clinicians for recommendations CCS staff Peer families Schedule an appointment with a physician that may be qualified to provide healthcare to your child

Helpful information Start the dialogue with your child’s team Know your health care options Inquire about a Conservatorship before your youth turns 18 if necessary Find out if your child qualifies for SSI
