MAP Months’ Activities July/August Relaunch in ED as multidisciplinary tool –Inclusion in admission pack –Encourage ED staff to document admission history on MAP & raise issues MO training on ‘How to MAP’ Pilot RN training on taking a medication history Promotion via inservices, posters, audits

Medication-Related Issues Issues identified by ALL clinicians are noted on the front page Requires daily review during patient review Document, sign & date when actioned, or provide detail as to why not

Expect to see less of this… A valid suggestion, but… No name of contact person, in case feedback is needed Not ‘formally documented’: no record of intervention Post-it can easily be lost Could have been written on the MAP…
