2. SUBMISSION OF COMMISSIONING PACKAGES u All Commissioning Packages must be submitted to MCRC 4 months prior to the date of appointment via mail. u All Commissioning Packages must have all needed documents prior to mailing to MCRC.

3. MIDSHIPMAN GROUND COMMISSIONING PACKAGE u This type of commissioning package should be submitted for a midshipman who do not wish to pursue an aviation guarantee. u The student will be commissioned with a MOS of 9901.

4. NEEDED DOCUMENTS FOR A MIDSHIPMAN GROUND COMMISSIONING PACKAGE u Rough Appointment Acceptance Record (NAVMC 763) u Commanding Officer’s Endorsement u A signed request from the student u BUMED approved physical (that includes the BUMED letter and a copy of the physical and supporting documents that were submitted to BUMED for approval) u Current transcripts u All enlistment contracts (DD Form 4) u NROTC Scholarship or College Program Agreement u Birth Certificate u NROTC Cost Sheets u Security Clearance

5. MIDSHIPMAN AVIATION COMMISSIONING PACKAGE u This type of commissioning package should be submitted for a midshipman who was granted an aviation guarantee prior to submission of commissioning package. u The midshipman should already have an aviation guarantee letter. u The midshipman will be commissioned with a MOS of 7599 or 7580.

MIDSHIPMAN GROUND COMMISSIONING PACKAGE with AVIATION REQUEST MIDSHIPMAN GROUND COMMISSIONING PACKAGE with AVIATION REQUEST u This type of commissioning package should be submitted for a senior who has not been granted an aviation guarantee prior to submission of commissioning package and desires a guarantee. u A senior can request for aviation at the time of commissioning ONLY!! u The student will be commissioned with a MOS of 9901, 7599 or 7580. u If the aviation request is approved, the student will not receive any notification from MCRC, he or she will receive commissioning documents with the MOS of 7599 or 7580.
