SDS ACCOMMODATE: Student Portal Semester Request Form ( Request for Classroom Accommodations ) Student Disability Services

The Student Portal can be accessed by clicking this link: https://southalabama- accommodate.symplicity.com/students/https://southalabama- accommodate.symplicity.com/students/ The log in page looks like :

The Semester Request form can be found under the Accommodation tab.

To begin the Semester Accommodation Request, click “Request Accommodation” at bottom left of screen.

NEXT STEP: 1. Select the accommodation that you want to receive to be listed on your letter. Only your approved accommodations will be listed. The system will require you to address each accommodation separately. You will have the option to request additional accommodations.

2. Select the semester for which you are completing the form.

3. Select the course(s) for which you would the accommodation.

4. If you are requesting more than one accommodation, select “Request Additional Accommodation” at the bottom of the form.

You will need to repeat steps 1-4 for each requested accommodation.

5. Click on Submit to complete your Semester Request 6. Once you have completed all the steps you should receive a confirmation email stating that your request was submitted.
