All TO BE SCORED Test Materials WHITE Boxes All NOT TO BE SCORED Test Materials Brown Boxes Large Print/Braille Brown Boxes Oversized Brown Boxes (If applicable) District Coordinator Box Create a Label *It is your responsibility to package the materials correctly for processing and score reporting. Delays or errors in packing may cause delays in processing information and the delivery of score reports. You Will Be Preparing The Following Types Of Boxes For Return To The District Coordinator:

Preparing To Return Test Materials All materials must be returned to ETS Organize your boxes: White Boxes Assemble all the White Boxes Brown Boxes Gather the Brown Boxes Brown Boxes Locate the return shipping labels for the Brown Boxes that were included in your shipment Brown Boxes Gather the Large Print/Braille oversized Brown Boxes

___1. Verify that all distributed secure materials have been returned. Notify the District Coordinator immediately if any secure materials are missing and complete the necessary investigation. ___ 2. Complete the CELLA Record of Exemptions, Absences, and Invalidations. Keep a copy of this record for your files. ___ 3. Make a copy and file the Test Administrator Security Log. ___ 4. Verify that the 2007 Florida CELLA Test Administrator’s Suggestion Forms are completed. ___ 5. Organize all of your TO BE SCORED materials by grade level. As You Receive CELLA TEST MATERIALS from The Test Administrators, Do The Following: The Test Administrators, Do The Following :

Take all materials TO BE SCORED for one grade level and stack them together. Pg. 36 For Grades K-2, you will only have Level A Student Books. For Grades 3-8, you may have a combination of Level A Reading and Writing Books and Level B, C, and D Answer Sheets. For Grades 9-12, you will only have Level B, C, and D Answer Sheets. Complete a pre-slugged Grade Header Sheet for each grade tested. You will need to complete items 4, 5, and 6 on the Header Sheet. Pg. 37 If you do not have the pre-slugged Grade Header Sheet, you will need to write and bubble in all information for items 1 through 6. Once you have completed the Grade Header Sheet for each grade tested, place it on top of the corresponding stack of TO BE SCORED materials. Wrap paper bands around each stack to ensure it stays together. If you do not have enough paper bands, just stack the materials and send them back un-banded. Pg. 38 White Boxes Place all TO BE SCORED material in the White Boxes making sure that the CELLA School Header Sheet is completed and placed on top. White Boxes Number all White Boxes 1 of X, 2 of X. Brown Boxes Do not include the Brown Boxes in this count. Do not include the Large Print/Braille boxes in this count. As You Receive CELLA TEST MATERIALS From The Test Administrators, Do The Following: Large Print/Braille Material 1.Gather all of the Large Print/Braille materials. 2.Place all Large Print/Braille materials in the boxes that they were sent in. 3.When packing the box, place the unused materials on the bottom and any used materials on the top. Brown 4.Take one of the Return Address Tracking Labels and place one label on each Brown oversized box. These labels will be used when shipping the boxes to ETS. Do not alter these labels in any way. 5.Number the boxes 1 of X, 2 of X. White Boxes Do not include the White Boxes in this count. Brown Boxes Do not include the Brown Boxes in this count.

FINAL PACKING INSTRUCTIONS 1. Complete the School Materials Return List located in the 2007 Return Instructions for School Coordinators. WhiteBrown √ Be sure to record the total number of White, Brown, and Large Print/Braille boxes for your school in the space provided. √ Be sure to check off each grade tested in your school. 2. Place the School Materials Return List in Box One of the White Boxes White Boxes and seal. 3. Seal all other boxes. May 16, 2007 4. Send to District Warehouse by May 16, 2007.

As You Receive CELLA TEST MATERIALS From The Test Administrators, Do The Following: 1. Gather all NOT TO BE SCORED materials. This will include all: Test books Unused Level A answer books Unused answer sheets Directions for Administration and Scoring Guide Test Administration Manuals (gray or pink) Brown Box 2. Complete the NOT TO BE SCORED School Header Sheet and place it on top in the first Brown Box. Brown Boxes. 3. Place all NOT TO BE SCORED materials in the Brown Boxes. NOT TO BE SCORED or unused answer sheets and Level A answer books should be sorted together and returned in the first Brown Box.

As You Receive CELLA TEST MATERIALS From The Test Administrators, Do The Following: Brown Box 4.Take one of the Return address tracking labels retrieved from the box when received and place one label on top of each Brown Box. These labels will be used when shipping the boxes to ETS. Do not alter these labels in any way. Brown Boxes 5.Number all Brown Boxes 1 of X, 2 of X. White BoxesDo not include the White Boxes in this count. Do not include the Large Print/Braille boxes in this count.

Damaged Answer Documents If an answer document is damaged either from the initial packaging of the documents or through handling during test administration: For example: torn, folded, marked by a pen, etc. The School Coordinator needs to transcribe all the information from the damaged answer document onto a new document. Brown Box The damaged answer document must be returned to ETS marked with a large “X” and returned in the Brown Box with the NOT TO BE SCORED materials.

Damaged Answer Documents Damaged Answer Documents (Continued) If due to a student’s illness: The School Coordinator should attempt to transcribe the student’s responses onto a new answer document. If unable to transcribe, a make-up date within the testing window should be arranged.

Miscellaneous Forms 1. 2007 Florida CELLA Test Irregularities Report Form White Box Place all completed forms in the top of the first White Box under the Materials Return List. 2. 2007 CELLA Administration Survey FAX to ETS at 866-387-2598 or place all completed forms in the top of the first White Box White Box. 3. 2007 Florida CELLA School Coordinator’s or Test Administrator’s Suggestion Form Place all completed forms in the top of the box in which you are returning unused answer sheets. 4.2007 CELLA Test Question Ambiguity and Error Form Place all completed forms in the top of the box in which you are returning unused answer sheets.
