3 Information obtained from distribution and collection of 2 questionnaires at 2 worship services. (198 yielded) majority of respondents indicated they seek medical care and follow prescribed treatment plans 1/3 of respondents correlate their spiritual life with health maintenance respondents have ideas about what they need to do for exercise and weight control but lack motivation to carry out the activities there is some conflicts about eating well and should eat better only 64 respondents said they eat well (Hickman, 2006, pg. 106)

12 Implementation: Individually based on needs after self assessment/health screenings Review and set individual goals Provide and ensure adequate education/tools available Host activities and continuing educational events within community and congregation Provide ongoing support community, congregation, ministry team, national resources, online support Evaluation: 3 and 6 month post assessment/health screenings individually 12 month re-evaluation of congregational needs (use original questionnaire for comparison) Feedback from members personally as well as anonymous through mailings sent by Parish Nurse
