It has been identified that there is a commissioning gap which the CCG are seeking to fill ASAP. QOF QP 2013/14The QOF QP pathways are: Outpatient Referrals; ALL, OA Hip and Clinical Thresholds. Emergency Admissions; ACE3 tool, Admission Prevention Services and Diabetes A&E attendances; Practices will need to develop and implement an improvement plan. The briefing for this is included in the delegate packs.england.ea-QOF@nhs.net NHS 111 UpdateFollowing national feedback, the number of duplicate messages to GPs from 111 have reduced, which will cut down on paperwork in surgeries. The secondary post-event messages, sent when a patient is referred to a GP OOH service to reduce message duplication, has now been supressed. These can be found on our website and samples are included within your pack NSFT Access and Assessment Service – tracking the referral process The performance of the contract is under review. A remedial action plan has been put in place with a goal to meet targets by the end of March. A workshop has been arranged to understand the causes better and to explore options for further improvements. A similar situation exists with the Wellbeing Service line which will also be explored. Individual organisations’ and system-wide action is having really positive results. Ipswich Hospital has extended services over the weekends with increased presence of senior clinical decision makers. The A&E department has consistently achieved >95% on their 4 hour target since Christmas Eve and is currently achieving 97.56% for the month of January. 24hr ECGThe implementation plan with practices which have expressed an interest in the contract is being finalised. Costings for 2014/15 are being reviewed and will be discussed with all practices and the LMC.

Issue/QueryOutcome/Update Emergency PlanningThe CCG is liaising at Chief Officer level to clarify CCG and Area Team responsibilities in supporting practices in developing and implementing emergency plans 5 year and 2 year planning In line with national guidance, the CCG is developing plans which will include; 1) major work programmes of the transformation of the urgent care system (discussed at December event), 2) efficient and effective working of Ipswich Hospital and West Suffolk Hospital and 3) healthy independent people working with Suffolk County Council
