6. Additional Reminder Completeness of documents and consistency of the info in all requirements should be met. Company Name and name of Rural Bank should be the same in all documents including Articles of Incorporation, MSA, SEC Registration, Settlement Account, Bank Application to Offer GCash, Billing Statement RB only needs to forward to GXI, a copy/proof showing that the settlement bank account nominated is named after the Rural Bank.

7. Additional Reminder On MSAs, indicate the list of branches that will use GCash and its preferred billing address. Only those included in the list of branches will be accredited. Should the RB opt to add some more branches, another MSA indicating the names of the new branches should be forwarded to GXI. On MSAs, RB should indicate if RB is requesting for a postpaid plan with a free handset. There is no more Plan 200 for the RBs. The price of the free handset will now determine the amount of their monthly postpaid plan.

8. Additional Reminder 2 copies of notarized Sec Cert All names of authorized signatories should be declared in the Sec Cert. In the Sec Cert, it should indicate the number of signatories that RB is authorizing to sign, execute and deliver in behalf of RB. This should be consistent with the number of signatures that will appear in all documents passed by RB to GXI. All authorized signatories (listed in Sec Cert) should provide individually a valid ID.
