Numbers 787 people on the GP’s dementia registers Expected prevalence of 1651 Therefore only 47.7% have a diagnosis In comparison with other areas we are 73 out of 178

Diagnosis Only 47.7% of people with dementia have a formal diagnosis Middlesbrough fits the national picture Early diagnosis means a better prognosis

What we are doing to improve the diagnosis rate Raising awareness through the response the Prime Minister’s Challenge on Dementia The Mayor launched Middlesbrough’s response to the challenge in dementia awareness week Identifying GP surgeries who are less likely to refer

Diagnosis Memory clinic - TEVW Currently diagnose people within 6 – 8 weeks 1 – 2 weeks – initial assessment 2 – 6 weeks – CT scan 6 – 8 weeks – diagnostic appointment

Support post diagnosis Medication – majority of people are prescribed medication. Those who are not usually have heart problems or low blood pressure. Memory clinic will work with GPs to manage physical problems to enable medication to be prescribed.

Support post diagnosis cont… Social care Support services – Sanctuary Advice re. DVLA Advice re. finances e.g. Lasting Power of Attorney

‘This is me’ Completing a ‘pen picture’ of that person that they can take with them wherever they go to remind people who they are, their likes and dislikes and wishes and feelings.

As the illness progresses Intensive Community Liaison Service – TEWV Work with people to enable them to remain at home for as long as possible. Work with care homes to provide the best care and treatment

South Tees Foundation Trust Launching the dementia strategy in July 2013 and the priorities are: Information Improving care Education Developing partnerships Improving environments so that South Tees will become a dementia friendly hospital Staff awareness and training

What we still need to do Make Middlesbrough a dementia friendly community e.g. shops, transport etc Increase awareness so that people will go to their GP Improve end of life care for people with dementia
