Evolution of Public Policy and Aging Economic and political forces, massive unemployment, poverty, and a significant increase in persons 60 and older, contributed to the establishment of the SOCIAL SECURITY ACT of 1935 1950 First National Conference on Aging – 816 delegates – America learned that older people existed and their needs differed from other age groups 1961 WHITE HOUSE CONFERENCE ON AGING – 2,500 delegates – central focus was health care 1965 Medicare, Medicaid, and Older Americans Act enacted by Congress White House Conferences on Aging – 1971, 1981, 1995, and 2005 Changes in Aging policy have followed the White House Conferences on Aging in terms of Older Americans Act and Social Security & Medicare

National Family Caregiver Support Program Information to caregivers about available services Assistance to caregivers in gaining access to supportive services Individual counseling, organization of support groups, and caregiving training Respite care to enable caregivers to be temporarily relieved from their caregiving responsibilities Supplemental services, that complement the care provided by caregivers

Case Management Program for Frail Elders Comprehensive system that monitors patient care by an individual case manager Staffing Team is defined as the case manager, the client, and anyone the client wants included Client choice is a primary component of the long-term care continuum System incorporates: screening, assessment, written care plan, information, referral or service provision, care monitoring, on-going follow-up and reassessments, evaluation of services results, and discharge planning

Case Management Program for Frail Elders Assessment tools: FASE (Functional Abilities Screening Evaluation) and I-OASIS (Iowa Outcomes-based Assessment Information Set The Staffing Team works with the physician to develop and follow a care plan suited to the needs of each older patient Companion program – Medicaid Elderly Waiver (Title 19) which waives certain Medicaid statutory requirements to cover a wide range of home and community-based services as an alternative to institutionalization Redesigned program reimburses the administration of case management service through Medicaid (Title 19)
