CTC will build roster 4 weeks prior to each assessment window CTC will conference with student’s teachers Teachers will circle (yes or no), sign, and date that they have evidence of effectiveness for each student’s accommodation(s) assigned for state assessments If a teacher circles “no” campus can re-address accommodation’s effectiveness prior to E11 date If no evidence exists for an accommodation’s effectiveness, the committee involved must amend the student’s IEP/IAP to remove the accommodation for that assessment

A test irregularity is reported to you Assess the situation, investigate and contact the DTC If the irregularity is ruled an incident obtain statements, fill out incident form and submit both to the DTC Follow up with an action plan to correct the error Keep a copy of the incident form and statements in your testing binder Remain calm! Get the facts, secure any evidence and determine the necessary steps Scan statements and incident form to the DTC by the end of the day the incident occurred Statements must be typed, dated and signed Do not discuss with anyone until cleared by the DTC Use district action plan codes Testing binders are kept on campus for 5 years and are TEA auditable
