Online Surveys Purpose? To collect information from respondents Tool: Online survey tools –Free Online survey sites such as Survey Monkey Restricted features –Web 2.0 tools such as Google Apps Fully featured without the Glitz Reporting not as comprehensive

Survey Elements Relevancy 1.Questions are relevant to what you want to find out? 2.Are your respondents in a position to answer the questions? 3.Are you familiar with the questions? 4.Are you familiar with the objectives? 5.What information is needed?

Plan your survey Storyboard your survey on paper first, showing questions and question types etc..... This is easier than creating a survey on the fly....

Click on Documents to access Forms

Click on Create new : then select Form

Click here to give your form (survey) a title

First give your survey a title and information about your survey.

My first question is an example of a text based question. Respondents fill in their answer in a text box. I have provided some Help Text to assist in answering the question. I have also made this a required question. When satisfied click Done.

Fill in your questions and choices Note I am making each question a required question. That means the respondents cannot move on unless the question is answered. Then click Done.

You have three options: 1.Email a link to respondents 2.Place a link in a moodle web page 3.Embed a link in a moodle web page or label

Click on Email this form then enter email addresses Then click send.

To embed: click more actions and Embed

Copy the embed code: Control V

In a Moodle Block: click add a resource: insert a label

Click on Toggle HTML source button

Paste in the embed code and click the toggle button again

Click on Save and Return to course
