Radiology Packet 15 Nasal/Bullae

16-year old MN Siamese cat “Felix” Hx: 4 year history of nasal discharge – mostly right sided. He has had a right-sided head tilt for 1-2 years.

16-year old MN Siamese cat “Felix” RF –Bilateral thickening of the tympanic bulla. –There is a slight increased opacity of the tympanic bulla with perhaps a increased density in the area of the petrous temple bone. –There is a slight increased radiolucency present in the right nasal passage. – There is definitely asymmetry of the nasal passages. –There may be a slight irregularity and deviation of the vomer bone and nasal septum on the right side. –The upper right canine and all of the incisors are missing. –The animal is also missing most of its molars and premolars. RD –Thickening of both tympanic bulla –Turbinate destruction R/O –Chronic rhinitis –Neoplasia Next –Biopsy

12-year old MN DSH “Teddy” Hx: He is presented for evaluation of a large, hard swelling of the rostral right hemi-mandible. The swelling has been present for some time but recently increased rapidly in size.

12-year old MN DSH “Teddy” RF –There is a large area of proliferative bone at the rostral aspect of the right hemimandible. –A large area of bone destruction is present at the rostral aspect of the mass and extending rostrally to surround the root of the mandibular canine tooth. RD –Mixed proliferative and lytic lesion of the right hemimandible R/O –Neoplasia Next –Biopsy

8-year old FS Doberman Pinscher “Tara” Hx: Presented for evaluation of left-sided facial swelling. The swelling is rostral to the medial canthas of the eye. During PE it is noted that the dog has severe dental disease.

8-year old FS Doberman Pinscher “Tara” RF –There is loss of the lamina dura and resorption of bone surrounding the rostral and caudal roots of the 4 th maxillary premolar. The resorption is more obvious around the rostral root. RD –Abscessation of the rostral and caudal roots of the left maxillary 4 th premolar

13-year old English Springer “Lincoln” Hx: Has unilateral nasal discharge of one month duration.

13-year old English Springer “Lincoln” RF –The lateral view reveals an increased opacity in the area of the frontal sinuses. –There is an increased density in the caudal part of the nasal passages. –The frontal view shows an increased opacity of the left frontal sinus. –Open mouth VD view reveals an increased opacity involving the entire portion of the left nasal passage. –There is loss of normal turbinate pattern, both rostrally and caudally. –There is some erosion and appears to be some slight deviation of the vomer bone in the caudal portion of the nasal passage. RD –Increased radiopacity and loss of normal turbinate pattern of the left nasal passage R/O –Nasal tumor Adenocarcinoma Fibrosarcoma Chondrosarcoma Osteosarcoma

10-year old Samoyed Hx: Presented with epistaxis and nasal discharge.

10-year old Samoyed RF –There is increased opacity throughout the right nasal cavity. –Destruction of the dorsal concha and ethmoid turbinates is apparent. –Although there is no obvious disruption of the nasal septum there does appear to be an area of increased opacity in the mid-portion of the left nasal cavity. –There is increased opacity of the right frontal sinus. RD –Destructive process within the right nasal cavity with suspected extension to the left R/O –Neoplasia –Aggressive fungal rhinitis

5-year old MC Cocker Spaniel “Sydney” Hx: Chronic ear infections.

5-year old MC Cocker Spaniel “Sydney” RF –Complete obliteration of the external ear canals with soft tissue swelling, especially on the left. –Mineralization of the right ear canal. –The oblique and lateral views suggest soft tissue opacity within the tympanic bulla. The right bulla is mildly thickened. –The right petrous temporal bone appears more sclerotic than the left. RD –Bilateral otitis media and externa

2-year old cat “Winkle” Hx: Respiratory difficulty.

2-year old cat “Winkle” RF –There is a very large soft tissue mass which is displacing the common pharynx ventrally. –There appears to be stretching of the hyoid apparatus. –Megaesophagus. –Increased in density of the nasal passages. RD –Large retropharyngeal mass R/O –Nasal-pharyngeal polyp Next –Nasal and sinus examination under anesthesia once the airway restriction has been removed. –Barium swallow 7-10 days after removal of airway obstruction to evaluate esophagus.

12-year old Persian “Damien” Hx: Chronic sneezing and unilateral nasal discharge.

12-year old Persian “Damien” RF –Increased opacity of the right nasal cavity. –The normal turbinate pattern is lost. –The right frontal sinus is opacified. –Multiple teeth are missing. –The left nasal cavity and frontal sinus are normal by comparison. RD –Destructive process within the right nasal cavity. R/O –Inflammatory rhinitis –Neoplastic process Lymphosarcoma Next –Nasal biopsy

15-year old DSH Hx: The cat is dyspneic and the owner complains that the cat “makes too much noise while sleeping.” On PE a serous bilateral nasal discharge is noted. There is increased respiratory effort and the cat breathes through its mouth. Two views are provided. One was obtained with the animal awake and one with the animal anesthetized.

15-year old DSH RF –In the awake view there is marked dilation of the pharynx. –The trachea, especially in the cervical portion also appears to be dilated. –In both views there is a 9mm round soft-tissue mass in the rostral nasal pharynx. –The rostral margin of the mass is better seen in the anesthetized view. RD –Nasal pharyngeal mass R/O –Nasopharyngeal polyp –Neoplasia Next –Biopsy

5-year old Cocker Spaniel “Sandy” Hx: Presented today because of right-sided head tilt and foul smelling ears. He has a history of chronic ear infections.

5-year old Cocker Spaniel “Sandy” RF –There is mineralization of both external ear canals. The right ear canal is more extensive. –In the open-mouth view (basilar view) mild thickening of the right bulla is noted. –There is also increased opacity of the lumen of the right bulla. RD –Bilateral otitis externa (chronic/severe) –Right otitis media
