1. REAL TIME INTUBATION DATA COLLECTION

2.  Intubations in the ICU  Nationwide Multicenter study  Begins once the IRB has been approved in your center

3.  1. To describe the current intubation practice through a multicenter study.  2. To collect information on complications that may surface during the intubation process and to isolate predictive factors for these complications.

4.  Prospective cross-sectional, observational study of adult critically ill patients admitted to ICUs at participating centers.  MICU  SICU

5.  Population/setting: ◦ 10-25 ICUs (medical/surgical/mixed) will be participating in the study. ◦ 1000-2500 Patients will be included in the study.  Inclusion: ◦ All adults (>18 years) ◦ Patients admitted in ICUs (medical and surgical)  Exclusion: ◦ Patients <18 years of age ◦ Previously enrolled in this study ◦ Patients with endotracheal intubations performed outside the ICU (in the operating room, in and outside the hospital) are excluded.

6.  Respiratory Therapists/Nurse Managers/NP / NA/other designated person will complete the questionnaire form following the intubation.  The physician provider will assist with a portion of the questionaire.

7.  Physician provider will need to determine the following:  Complications  Lifting force  Position of vocal cords  Glottis exposure: (Cormack and Lehane grade)

8.  A completed forms folder will be returned to the HEMAIR study Investigator in the center.
