2. Self Referral Patient sees GP Matthew attends his GP surgery following a general health check for over 40s. Matthew smokes, is overweight, drinks too much and recent blood tests have shown a cholesterol level of 7.2. 1

3. Self Referral GP signposts information Dr Caroline Taylor, Matthew’s GP, tells him that unless he changes his lifestyle now, he is likely to suffer from some serious illnesses in the future. Matthew agrees to consider losing weight, stop smoking and cut down on the alcohol. Dr Taylor informs Matthew that there is lots of advice available on the internet and tells him that he can self-refer to a range of services. 2

4. Self Referral Patient views online Information Later that evening, Matthew logs on to the online patient service and finds a number of local services that can help him to stop smoking. He also finds some dietetics services. 3

5. Self Referral Patient self-refers Matthew selects a dietetics service, completes a self- referral form and is shown the available appointments. 4

6. Self Referral Patient attends Appointments Matthew attends both appointments and confirms that he is happy for the outcomes to be shared with his GP. 5

7. Self Referral GP receives outcomes Dr Taylor can see on her worklist that Matthew has followed her advice and attended two suitable clinics. Her clinical system is updated. 6
