Eligibility Criteria To apply for level 1, pharmacies must: Deliver tier 2 Stop Smoking Service at least one trained Stop Smoking Adviser, service available >80% of opening hours, must have set a minimum of 24 quit dates in the previous year Deliver Medicines Usage Reviews

Eligibility Criteria (cont) Deliver at least one of the following: –Supervised Methadone Available contracted hours –Minor Ailments Available contracted hours –NHS Health Checks Available 80% of opening hours and at least 12 NHS Health Checks 1 Oct 10 – 30 Sept 11 –EHC Available 80% of opening hours –Flu vacs available Be signed up to attend Royal Society of Public Health Training

Eligibility Criteria Level 2 (to follow) To apply for level 2, pharmacies must: Increase Stop Smoking Service - two trained advisers, quit rate of 45% since award of level 1 Increase EHC availability to 100% (exemptions will be considered for exceptional circumstances on application) Increase NHS Health Checks 100% availability and increase baseline activity by 10% since the award of level 1 Deliver three of the five optional services listed above at level 1
