Region 1 Lead Entity The East of the River Clergy Police Community Partnership, as the Lead Entity for wards 7 and 8 and parts of Prince George’s County, has been contracted by the Department of Youth Rehabilitation Services (DYRS) to serve as a broker, coordinator, funder, and capacity builder for the Rising Youth Coalition, a community-based continuum of care developed by the Lead Entity to service DYRS committed youth. ERCPCP ensures that the services, supports and opportunities that are identified in youth’s case plans through the collaborative and consensus-driven Youth Family Team Meeting (YFTM) are readily available and delivered, utilizing Positive Youth Development principles. The core values and guiding principles adopted by Wraparound Milwaukee and the Wayne County Michigan Department of Children and Family Services were infused into ERCPCP’s Lead Entity design.

Service Coalitions The Service Coalitions are collections of community-based organizations and grassroots providers that are members or affiliates of the Lead Entities Rising Youth Region I (East of the River) Wards 7 & 8 and parts of Prince George’s County Serving up to 385 youth and families Bridge Builders Region II (West of the River) Wards 1 through 6 Serving up to 290 youth and families

Service Referrals from DYRS Youth enter the Service Coalitions through Youth Family Team Meetings that are prioritized by DYRS according to : Youth newly committed to DYRS (initial YFTM mandatory) Youth currently placed in the community and in crisis and/or with a need for increased services Youth returning home from placements, including New Beginnings and RTCs Youth in the community
