APF Process Purpose: –Communication tool to ensure that the health care provider, injured worker, employer, and claim manager are all on the same page –Emphasis on early return to work opportunities and discussion of physical restrictions –Certifies time-loss Upon 1 st visit or when status/restrictions change Focus on timeliness and completeness
