Necessary to Complete Verification Physical Verification of Assets Update Necessary Data from Spreadsheets Return Signed Memo JHHS Disposition/Transfer Form All forms to be returned by June 5, 2009

Update Spreadsheet Below is an example of the asset spreadsheet. Update areas as necessary. Serial Number, CC, CES and Building can be updated.

NCB Check Box The NCB Check Box is an important item as we prepare for the NCB. Use this box to indicate if the asset will be used/moved to the NCB.

Return Signed Memo The FA Verification Memo needs to be signed and returned to the FASSC by the due date of June 5, 2009. Sign and return regardless of changes. Sign and return. Fax to 443-997- 5728 or Email to FASSC

JHHS Disposition and Transfer Form Fill out the Disposition/Transfer Form if applicable
