1. THERAMAX THERAPY COMPANY INTAKE/PATIENT REFERRALS  Objective: To be able to level off how to handle referrals for TTS Pediatrics appropriately and make decisions on a timely manner whether to accept referrals or not.

2. UPON RECEIPT OF REFERRALS  Write down all patient information to the Intake sheet.  Verify patient eligibility using THMP connect.  If patient is eligible, assign an Medical Record Number

3. Referral process cont:  Verify also MD eligibility: 1. Get correct NPI# of referring MD go to: https://nppes.cms.hhs.govhttps://nppes.cms.hhs.gov 2. Make sure to verify correct referring MD NPI# and check this against what is in Kinnser, if there is discrepancies, change the Kinnser information.

4. Referral process cont: 3. Verify also Physician against the oig exclusion list: go to oig.hhs.gov/exclusions/index.asp. If MD is not on the list go ahead to process patient data entry to Kinnser. 4. If Red Flag, MD is in the list- consult Admin what to do.

5. Referral process cont.  Enter pertaining information into Kinnser.  Make sure to double check all correct insurance info: ID#, SSS#, Age, correct name in the eligibility check.  Make sure to obtain authorization and enter it in Kinnser, make sure to input the date range.

6. Referral process cont.  Notify Scheduler for patient scheduling for Evaluation.  Make to obtain signed/dated MD order before assigning therapist to continue visits.  Obtain continuity of authorization if needed.
