2. Case Study 1 After 3 Months Intervention (2/08)  Bilateral HA on 1/27/08  Auditory Assessment on 2/10/08 No response to sound and speech w/o HA, Showed responses to loud indoor environmental sound and loud speech within 1-3 feet w/HA Initial LDS (2/10/08) and 2 nd LDS (5/14/08)- Receptive and Expressive Levels?  Audiological evaluation reports from AASD  ENT: Dr. Bauer at PENTA  BCW IFSP dated 11/6/07, YF received SLP, OT and PT services  Regular Georgia PINES visits with no cancellation  Child babbled and cried to indicate wants and needs  Mom wants to consider CI  MB CDI- Not applicable  Auditory-Verbal Development Checklist (2/10/08) showed no response to sounds  IPP should be in place

Case Study 1 After 9 Months Intervention (8/08)  Child is 13 months old  Auditory Assessment on 7/24/08- see evaluation No response to sound and speech w/o HA, Showed responses to loud indoor environmental sound and loud speech within 1-3 feet w/HA 3 rd LDS done on 8/10/08- homework assignment  Audiological report dated 4/11/08 from PENTA  ENT: Dr. Bauer at PENTA  BCW IFSP review dated 5/6/08, YF received SLP and PT services  Regular Georgia PINES visits with no cancellation  Child babbled and cried to indicate wants and needs  Mom learned signs for “finish”, “milk”, “eat”, “shoe”, and “more” from SLP.  Started L’TL Kids at AASD on 8/19/08  Evaluation report from L’TL Kids on 8/19/08  CI surgery was scheduled for 10/13/08  M-B CDI Words and Gestures completed on 8/17 /08, need to score  Auditory-Verbal Development Checklist done on 7/24/2008- used to develop auditory goals for IPP review.
