Introduction The purpose of this presentation, is not to review all the uploading readings for this module, but rather to guide your self study by discussing areas to focus on. The Module 6 folder contains all new materials regarding Diabetes Self Management Education & Teaching (DSME/T) but in order to review the other materials related to this topic you may need to revisit Modules 2 & 5. ADA Standards of Care in Diabetes 2014 Divided into 10 sections in Module 2. Review the sections of the updated guidelines as they relate to DSME. Section V. Diabetes Care & sections A-M are what pharmacists are expected to be able to educate a patient with diabetes on – not just medications! Section VI. Prevention & Management of Diabetes Complications reviews the ABCs of DM management, smoking cessation & both management of macro- & micro-vascular complications. Section X. Strategies for Improving Diabetes Care focusing Chronic Care model (CCM) & supporting behavior changes for the patient. Divided into 10 sections in Module 2. Review the sections of the updated guidelines as they relate to DSME. Section V. Diabetes Care & sections A-M are what pharmacists are expected to be able to educate a patient with diabetes on – not just medications! Section VI. VA/DoD Guidelines on Diabetes Management in Primary Care from 2010 Revisit Module 5 of this course for DSME from the VA/DoD Practice Guidelines. Make sure to review Modules D & M plus the algorithms for each module. Module D focuses on CORE principles of DM management recommendations. Module M focuses on DSME/MNT education & recommends using the Survival Skills Handbook for People with Diabetes plus refers you back to Module D to review content. Survival Skills Handbook for Persons with Diabetes teaching document – review the entire presentation and focus on the red flags. Revisit Module 5 of this course for DSME from the VA/DoD Practice Guidelines. Make sure to review Modules D & M plus the algorithms for each module. AADE: Competencies for Diabetes Educators Define the 5 levels of educators and be familiar with the differences in roles and responsibility/program management between the 3 level.

White Paper on Diabetes Literacy & Numeracy Be familiar with different tools utilized in DSME/T to evaluate the learning style of a person with diabetes.

National Standards for DSME/T & Insulin Education Review article provided on the National Standards for DSME & be familiar with the rationale behind the 10 defined standards. Review AADE White Paper on Insulin DSME and be familiar with the identified barriers to care & teaching strategies discussed to help manage special populations.
