Medical Paperwork Physical must occur within six months of initial practice. Make sure ALL sections of the physical form are filled out. You may COPY for the health center Medical History Questionnaire must be completed – questions related more to athletic injuries, etc Some teams required to do concussion testing upon arrival prior to practice Insurance Requirement TWO copies of insurance card Signed insurance form Sickle Cell Trait Testing Form Provide result of sickle cell test Decline test WEBSITE ***these must be completed annually

The NCAA conducts random drug testing each year on campus and possibly at championships All Student-Athletes required to submit to testing or lose eligibility ADHD medical exception: MUST provide significant documentation for an exception. Students allowed to participate while taking this medication, BUT should begin to acquire the information needed for an exception.

Representatives from each team Mission: The West Chester University Student Athlete Advisory Committee (SAAC) exists as the voice of the student-athletes as a liaison between all teams and administrators, to oversee the welfare of all student athletes, and to promote a positive student athlete image by being involved, communicative and providing service to the community.
