4 ORIGIN AND HISTORY OF THE KNIGHTS TEMPLAR EYE FOUNDATION, INC. ► THE KNIGHTS TEMPLAR EYE FOUNDATION, INC. WAS ESTABLISHED BY THE GRAND ENCAMPMENT OF KNIGHTS TEMPLAR IN 1955 AND INCORPORATED THE FOLLOWING YEAR ► EARLIEST GOAL WAS TO CORRECT STRABISMUS OR “CROSSED EYES” IN CHILDREN 4

5 ► SINCE ITS INCORPORATION IN 1956 OVER $142 MILLION HAS BEEN EXPENDED ON RESEARCH, EDUCATION, AND PATIENT CARE ► OVER $23 MILLION IN GRANTS HAVE BEEN AWARDED TO RESEARCHERS WORKING IN THE FIELDS OF OPHTHALMOLOGY AND OPHTHALMOLOGIC GENETICS 5

6 GOVERNANCE THE KNIGHTS TEMPLAR EYE FOUNDATION, IS A MARYLAND CORPORATION GOVERNED BY A BOARD OF TRUSTEES COMPRISED OF THE SIX ELECTED OFFICERS OF THE GRAND ENCAMPMENT OF KNIGHTS TEMPLAR, ALL PAST GRAND MASTERS OF THE GRAND ENCAMPMENT OF KNIGHTS TEMPLAR, AND SIX TRUSTEES-AT-LARGE ELECTED FROM AND BY THE MEMBERSHIP OF THE GRAND ENCAMPMENT OF KNIGHTS TEMPLAR FOR 9-YEAR TERMS 6
