5 What is a Dementia Capable Community? A dementia capable community is informed, safe and respectful of individuals with dementia and their families, provides supportive options, and fosters quality of life. 5

7 Four-Phase Community Toolkit Convene key community leaders and members to form an Action Team. Assess current strengths and gaps within the community.
