The Swedish Shoulder Arthroplasty Register An analyse of Revisions in the Register B Salomonsson MD PhD (1), A Nordquist MD PhD (2), H Rahme MD PhD (3) (1) Stockholm, (2) Malm ö, (3) Uppsala, Sweden.
2
2 Purpose: To analyse the revisions in the register based on: –Different diagnoses –Concepts of arthroplasty.
3
3 Background: The Swedish Shoulder Arthroplasty Register started in 1999 Including 2010 we have 8100 surgical procedures reported
4
4 Material: 7463 Primary procedures were reported 1999-2010 637 Revsions and other re-operations were reported 1999-2010 –This is 7.9% of the reports
5
5 Methods: Of the re-operations reported to the registry, more than 230 indexoperations are found in the registry. These are analysed regarding diagnose and procedure.
6
6 Results RA: Glenoid problems as a cause of revision. For Reumatoid Arthritis: –HSA (n = 634) there were 16 revisions (2,5%) –TSA (n = 192) there were 2 revisions (1%) Hemi Shoulder Arthroplasty (HSA) Total Shoulder Arthroplasty (TSA)
7
7 Results OA: Glenoid problems as a cause of revision. For Osteoarthrosis: –HSA (n = 1311) there were 36 revisions (2,8%) –TSA (n = 852) there were 3 revisions (0,4%) Hemi Shoulder Arthroplasty (HSA) Total Shoulder Arthroplasty (TSA)
8
8 Results Second. OA / OA: Survival curves Kaplan-Mayer –Reoperations after indexoperation 1999-2009
9
9 Results HSA / TSA: Survival curves Kaplan-Mayer –Reoperations after indexoperation 1999-2009
10
10 Results Fractures: Cause of revision of implant. For Fractures with Hemiarthroplasty (HSA): –HSA (n = 2097) –Glenoid problems: 6 revisions (0,02%) –Instability problem:15 revisions(0,07%) –Infection:10 revisions(0,05%)
11
11 Results Fracture 2w: Survival curves Kaplan-Mayer –Reoperations after indexoperation 1999-2009
12
12 Conclusions: The numbers are still small and few cases may change the results. Still there are differences in revision rate between different: –Concepts Hemi / Total –Diagnoses RA / OA Early / Late fracture arthroplasty
13
13 Conclusions: Shoulder Arthroplasty is a procedure with good long time survival of the implants. The register will have better opportunity to evaluate the results as the numbers increases over time.
