2 Introduction  What is PSMP?  Initial visit will have patients meet with Health Coaches for about 30 minutes -Stages of readiness and confidence scales -Identifying barriers -Setting weekly goals  Subsequent visits can be shorter. -Will be used for follow up (in office or by phone) -Review of weekly progress  HC will meet with providers periodically.

3 Research Purpose As a preliminary step to measure the impact of health coaching on behavior change, the purpose of this research was twofold: 1.To examine overall stages of readiness in four health improving behaviors among patients at risk of lifestyle associated chronic diseases.  Sub-questions: Avoiding foods high in fat, eating a diet high in fiber, exercising at least three times a week for 30 minutes, and reducing stress.

5 Method Type of study: This was a pre, descriptive study that measured patients’ stages of readiness in four health improving behaviors. Procedures: A total of 64 uninsured adult patients who visited a local clinic due to their chronic illnesses were asked by health care providers to complete a four-item modified Stages of Change questionnaire upon their arrival at the clinic. Data Analysis: Respondents’ stages of readiness were classified into five categories according to the Stages of Change Model: pre-contemplation, contemplation, preparation, action, and maintenance.
