1 Recovery Centers of Arkansas WILD HOG OFFENSE Continuing Care As presented by “Coach” Rocky Halter

2 AIM (Plan) Team huddle Repeated Exposure Talk up continuing care as an option for everyone receiving services at the residential treatment level.

3 Change (Do) Execute Play (WILD HOG) –Centers – Screening personnel describe Continuing Care prior to admission for services; they identify what may be blocking strategies to protect client –Quarterback – Primary counselor during course of treatment and Continuing Care; helps provide direction, encouragement, and a sense of stability –Tackles & Guards – Reference is made to Continuing Care in the course of educational classes; addresses blocking barriers identified by client At the snap, the team pulls together with the goal of having Continuing Care included in the master plan of care developed by the client.

4 RESULTS (study) Out of 309 plays, there were 43 touchdowns Outcomes for this season were compared to outcomes from the previous season when Continuing Care was not available – this season 81% of clients 6 months post admission were free from drug use. Last season only 79% were clean. Continuing Care Wins !!!

5 NEXT STEPS (Act) With this information, the team decided to expand the use of the CC offense to those completing outpatient treatment.

6 IMPACT (Business Case, Lessons Learned) A fumble by one of the players must be recovered quickly (staff turnover resulted in a sharp decrease at one point during implementation) Fans are essential (easy to lose momentum without continued cheering) The bucket of ice water feels really good!!!
