Worked together for a year to develop a framework for falls prevention in Mississauga Halton and a companion resource guide. Purpose of the framework is to guide, inform and connect efforts across an integrated continuum of services.

Goal: Reduce falls and falls related injuries in older adults. Reduce the rate of emergency department visits, hospitalizations and long term care admissions among seniors in the Mississauga Halton LHIN.

Enhancement of Hospital Falls Prevention Clinic – Credit Valley Hospital Falls Clinics at Halton Healthcare and Trillium Health Centre. Home Support Exercise Program Stand – Up program Mississauga Halton Falls Prevention Coordinator role.

Credit Valley Clinic became the model for the others A lot of work went into documentation of program and processes Lessons learned ◦ Systems, skills and capacity for expected outcomes may not be in place. ◦ Program improvements are ongoing. ◦ LHIN expectations and reporting

10 simple exercises for frail seniors Funding was to implement via personal support workers ◦ CCAC clients & one supportive housing agency Lessons learned Don’t put all your eggs in 2 baskets It takes a long time for uptake in these environments Engage others who work with Seniors

Community based initiative Plan was to implement with parks and recreation. Lessons learned Make sure your partners can implement Find out what they need to contribute to falls prevention. Be flexible and advocate for changes

Connect with all who service seniors Ask for participation Show them how they can participate Connect the dots for everyone – and do it often Share what you know Don’t waste time trying to engage those who are not interested. Be flexible
