2 COMMUNITY FIRST OLMSTEAD PLAN ■Established 2008 by the Patrick Administration ■The Money Follows the Person (MFP) Demonstration Grant uses “Community First” as its foundation ■Vision Statement: Empower and support people with disabilities and elders to live with dignity and independence in the community by expanding, strengthening, and integrating systems of community-based long-term supports that are person-centered, high in quality and provide optimal choice.

3 COMMUNITY FIRST OBJECTIVES 1. Help individuals transition from facility based long term care 2. Expand access to community-based long-term supports 3. Improve the capacity and quality of community-based long-term supports 4. Expand access to affordable and accessible housing with supports 5. Promote employment and other meaningful community engagement activities for adults with disabilities and elders 6. Promote awareness of the full range of community long- term supports (LTS)

44 MFP OVERVIEW CMS awarded Massachusetts a 5 year grant in April 2011 Grant allows Massachusetts to: Transition elders and adults with disabilities into homes in the community Expand its existing commitment to support community living Rebalance the State’s Long Term Services and Supports (LTSS) system Procure and contract with 5 Money Follows the Person Regional Coordinating Offices (MFP RCO) that will provide MFP Demonstration Functions and MFP Waiver Functions 4

555 WHO CAN PARTICIPATE? Adults: 18 years of age or older Who are Medicaid eligible Who are in or will be in a MFP Qualified Facility for 90 consecutive days Who wish to participate in the Demonstration and transition to a qualified residence and sign an MFP Informed Consent

666 QUALIFIED FACILITIES DPH licensed and Medicaid certified nursing facilities (NFs) Chronic Disease and Rehabilitation (CDR) Hospitals Department of Public Health (DPH) Hospitals Intermediate Care Facilities for people with mental retardation (ICF/MRs) Institutes of mental diseases (IMDs)

77 QUALIFIED COMMUNITY RESIDENCES Home/apartment owned or leased Community-based residential home – for no more than 4 individuals Assisted Living Facilities meeting specific criteria

8 SERVING MFP PARTICIPANTS ■During Phase 1 MFP Participants will be served through EXISTING waivers and State Plan Services ■During Phase 2 beginning in the fall 2012 MFP Participants will be served through 2 new 1915(c) Waivers 1 new 1915(b) Waiver Along with existing waiver and state plan services

999 DEMONSTRATION FEATURES AND SERVICES Demonstration Features: Informed Consent Individualized Person Centered Transition Planning Availability of individualized 24 hour back-up plan Access to housing search assistance and coordination Risk Assessment Quality of Life Surveys Demonstration Service: Demonstration Case Management Assistive Technology Transitional Assistance Services Access to housing search assistance and coordination Orientation and Mobility Training

10 HOUSING SEARCH MFP Enrollees and Participants will be provided housing search assistance including the availability of funding for home and environmental modifications in public and private housing units; MRC will qualify Housing Search Entities (HSEs) across the state; MFP RCOs shall be required to have a Regional Housing Coordinator on staff that will: Develop affordable and accessible housing opportunities for MFP Participants in their designated region Track and report on number of affordable and accessible housing units available in he designated region to meet MFP target goals Ensure identified units meet definition of qualified residence 10

11 NEW 1915 (b) WAIVER ■EOHHS has submitted to CMS a Behavioral Health Waiver ■This managed care waiver would enable MFP Demonstration and Waiver Enrollees and Participants access to a full range of behavioral health services including outpatient, inpatient, 24 hour diversionary and emergency services

12 NEW MFP 1915 (C) WAIVERS EOHHS has submitted for approval two new 1915 (c) HCBS waivers MFP Community Living waiver - for individuals who do not need 24 hour supports or supervision MFP Residential Supports waiver - for individuals who require supervision and staffing 24 hours/day, 7days/week and receive services in a provider-operated and staffed setting The waivers will provide HCBS to help MFP-eligible persons move from nursing facilities, chronic, rehabilitation or psychiatric hospitals to the community
