LTI 011 Crushed Finger 13/May/2001 Crew were removing earth from bund wall area using a large metal skip. Position of skip had to be changed to help filling.

LTI 011 Crushed Finger 13/May/2001 Labourer tried to move bucket on his own. When he lifted the skip handle it crushed his hand that was positioned on the rim of the skip, severely injuring the little finger, top portion had to be removed

LTI 011 Crushed Finger 13/May/2001 What happened What happened? Crane positioned skip next to earth. Filling the skip was difficult because it was in the wrong place. Local supervisor was not paying attention to the work. Labourer decided to move the skip on his own. He positioned his hands without thinking about possible hazards. Finger was trapped between lifting handle and rim of large steel bucket. Top section was crushed.

LTI 011 Crushed Finger 13/May/2001 How do we put it right? Ensure all workers are properly trained for their work and are familiar with routine procedures. Ensure all work is adequately supervised. Before work starts supervisor must always hold a Tool Box Talk with men doing the job. Discuss all the possible hazards and how to control them. Stick to the Rules. STOP Work if Unsafe When doing the job Pay Attention at ALL times. Remember: Competence Compliance Concentrarion
