Transition Coordinator Responsibilities Determined by your SE Director & structure of your transition system Develop a system for reporting out to district staff- via director, email, department meetings, etc. May attend IEPs or may provide support to your middle school/high school/post high staffs Share transition resources with your district

Transition Coordinator CIMS Responsibilities may Include: Support for transition indicators: – B13 Secondary Transition Coordinate/organize the annual data collection Provide updates to director Provide training for staff – B14 Post School Outcomes Provide updates to director Provide training for staff Collect the alternative contact information for Wayne State University/ survey – B1Graduation/B2Drop Out May participate in school improvement team supporting graduation initiatives Assist in data review Provide great transition support so students stay in school and graduate!
