4. Service Provision From previous feedback & our own reviews we check more now, but we still need to build in more rigorous checking. 3 new forms will enable this. Revision Form – Support Staff Revision Form – Teacher New Starter Form – Support Staff & Teachers – Checked at start of process now

5. CRB Middle names on driving licence are required on all applications Driving licence photo card and paper counter part must be seen together as evidence ID checking is due to change on 1 st September 2012. Need to see passport, birth certificate and two proofs of current address issued within the last three months

7. Timesheets Teachers – Additional hours - HED 154 – Unpaid Leave - HED 062 (-) minus hours Non Teaching Staff – Additional hours - HED 064 or Overtime for a full timer – Unpaid Leave - HED 066 (-) minus hours

8. Timesheets General – Please use this form for all payments including Lunchtime supervision hours. – Please add additional hours / payments to the end of the sheet - do not delete or insert lines. – Should you need to add information after sending - please add the information and resend with a message asking for the original to be deleted. – Please let us know if you would like a visit regarding the monthly input.

9. Performance Appraisal Guidance DFE issued new statutory guidance on appraisals for teaching staff. Service Provision New Forms – Benefits - accurate, quicker production of contracts, will contact you just to check - so we get it right – Fill in white bits, the form will populate in yellow areas – We follow checking process when we receive it – New way of working for us / you, but overtime our records will improve.

11. Service Provision We will put new forms on extranet & send them via USOFX to you. Please start using them from when you receive them. If you have sent anything before today we will still accept it. After this week we will return it.

12. Service Provision New Starter Form is like this, it is a combination of current 2 forms. Send when all information is complete. We cannot put people on payroll until it is complete. Therefore do not send part completed ones Like now we need both parts of the form completed to pay people. We will send back part completed ones.

13. Service Provision September – We will use the new forms & send contracts out before the school holidays where we can – We will put everyone on payroll (but not pay them!), then send you a report in August (while you are off), to confirm details before we run the September payroll – Don't worry - we will take new starters / pay changes in September but we want to reduce the pressure / risk of errors.
