13 How can you access us? One referral form for IANDS The central referral team will triage all referrals through a single point into the most appropriate service Most referrals are made by SENCOs, GPs, paediatricians and hospital ward staff Parents and clients are not able to self-refer

14 How can you access us? Speech and Language Therapy Majority of referrals to our service come from the school –School complete a ‘Quick Communication Checklist’ which helps us identify specifically the concerns –School discuss the potential referral with their link SLT –The link SLT then advises whether a referral is indicated. There may be times where the SLT suggests alternative action (i.e. referral to another agency, suggestions for school-based support) We do sometimes receive referrals from other professionals, such as GP, Paediatrician, health visitor, OT etc –In these instances, we make contact with the school to gather information regarding the child’s communication skills in the school context.

15 When do you leave us? Children will be discharged from the service for the following reasons: –Therapy goals are achieved –Ongoing support can be carried out at home/school independently –Transition to adult services –Transition to another service –Non-attendance of appointments –Non-response to request for further information Families and GP will receive a discharge letter with summary of intervention, advice and re-referral details if necessary
