First ACTS: Creating an Incident Report First Student users who can create Incident Reports in First ACTS are called Writers. First ACTS supports two types of Writers (outlined below). Locations can have one or both of the following types of Writers: Incident Reports are released directly to the schools Incident Reports require FS review before they are released to the schools FS Writer creates Report Report sent to School for review FS Writer creates Report Report reviewed at FS Report sent to School for review This guide focuses on this scenario

First ACTS: Reviewing an Incident Report - 1 When an Incident Report is created that requires a FS review, the FS Reviewer(s) will receive an email notification from the system like the following: As the above message outlines, to open the Incident Report, the School Reviewer can simply click on the link in the message. This will take the Reviewer to the First ACTS login screen Using you First ACTS Username and Password, log into system

First ACTS: Reviewing an Incident Report - 2 1.Once logged into the system and the Incident Report requiring review is open, FS Reviewer can update or modify any incident information as needed To edit any fields, click on the pencil icon. To save edits, click on save icon. 2.When review is complete, FS Reviewer has three options: Approve and close – will complete the incident but not submit to school Reject – will remove Incident Report from system Approve – will submit Incident Report to school for review 1. 2.

First ACTS: Acknowledging an Incident Report School Reviewer, after reviewing the Incident Report and deciding any Disciplinary Actions, may check a box (see 1.) to notify FS of any decisions. Again, FS Reviewer will receive an email notification and, once logged into the system, will be able to view the school’s Disciplinary Action for the incident FS Reviewer can enter any Closing Notes (see 2.) and close the incident 2. 1.
