CHANGES TO STATE AT DATA REPORT – RSA 572 SFA – Financial Loan Deleted Active Loan Number and Active Loan Amount Deleted ratio calculations since active loan denominator is deleted. Will calculate ratio based on a sum of loans made (maybe prior 5 year aggregate or similar) Anecdotes (4 State Level Activities) Added check box to indicate picture is available Reduced text box display to encourage brevity Training and TA Narratives Reduced text box display to encourage brevity

PERFORMANCE MEASURE REVISIONS SFA and Reuse - Acquisition No change to acquisition measure itself or questions Added text box for required description of any exclusions to ensure accurate and consistent. Device Loan and Demo New acquisition measure for non decision-making loan purposes (repair/waiting for funding, short-term accommodation, professional development activities) Eliminate IT/Telecom area in both Loan and Demo Training New performance measure for IT training – outcome/ result from IT training received collected from participants

DEVICE LOAN ACQUISITION PERFORMANCE MEASURE Why did the individual borrow the device? * I could only afford to borrow the AT through this program. (I could not afford to rent or borrow it through other programs.) * The AT was only available to me through this program. (I am not eligible or there is no other rental or device loan program available; or the specific device I needed is not loaned by other programs.) * The AT was available to borrow from other programs, but the system was too complex or the wait time was too long. (I couldn’t borrow the device in a timely manner.) * None of the above

TRAINING PERFORMANCE MEASURE What is the primary outcome of participation in the training on Information Technology and Telecommunications Access? * IT and telecommunications (web, software, etc.) procurement or development policies, procedures, or practices will be improved or better implemented to ensure accessibility * Training or technical assistance will be developed/implemented to ensure accessibility of websites, software or other IT and telecommunications (web, software, etc.) * Outcome is unknown at this time.

CHANGES EFFECTIVE FOR FY15 DATA Data collected as of Oct 1, 2014 will use the new form. You may need to think about “carryover” data, e.g. device loan that begins in August 2014, but is not completed until end of October. Later discussion of IT/Telecom training capacity – is there interest in national online training? Will your data system be ready for these changes?

WHY DISCUSS THE ANNUAL REPORT TOOL? In a November 2013 survey of CATADA.info users, we discovered the following: 41% of survey respondents state they produce an annual report for their program based on their RSA 572 data The remaining 59% DO NOT produce a report Reasons given for not producing a report were:
