3  The 16 Alliances in Aligning Forces for Quality serve demographically, economically, and geographically diverse communities.  From the sparsely populated Humboldt County to the whole state of Maine, Alliances are multi-stakeholder coalitions focused on lifting the quality of health care in their region.

4  Self selected 8 AF4Q Alliances with mature or developing practice coaching programs  Affinity Group Aim Statement: ◦ Identify, highlight and share best practices and tools from the field featuring practice coaching strategies from inside alliance and outside experts.  Monthly webinars focused on practical applications of tools, resources and case studies for practice coaches and practice coach supervisors

8  Cohorts of small groups will ‘travel together’ to learn from the tool expert and learn from one another  Our Hypotheses: ◦ Small groups foster better understanding of the tools ◦ ‘Cohorts’ create opportunities to improve networking ◦ Active movement will lessen conference fatigue This is a P-D-S-A !!

9  Shot Gun Method (used in golf tournaments to allow play simultaneously)  Participants are assigned to small groups that start at a specified hole and move through the course (tool stations) together  Look at your number given to you upon entry  Find the golf hole associated with number  Group will be given 15 minutes per hole  Groups stay together, advancing to the next tool station

10 Kate has over 30 years of business experience that she brings to her role as Director of Regional Quality Improvement for the P 2 Collaborative of WNY. She uses her background in sales and engineering management within the manufacturing sector in combination with her training as a certified coach and team coach to support ambulatory quality improvement initiatives for P 2 throughout the eight counties of Western New York. She oversees a network of quality improvement practice facilitators and engages personally in conducting team coaching at the practice level to help primary care practices work through the stages of change necessary to support ongoing quality improvements.

11 Utilizing a diverse background of over 15 years of experience in customer service, IT, and project management in business and industry, Valerie works with healthcare teams to develop a team- based approach to process improvement. As a Dartmouth Institute-trained Clinical Microsystems Coach and certified TeamSTEPPS trainer, Valerie utilizes a variety of tools to help teams create sustainable systems, improve outcomes and increase patient/staff satisfaction.

12 Rush has over 18 years of experience in production, management, project management and quality improvement at General Mills, Snyder’s of Hanover, Frito-Lay and Starbucks before venturing into the healthcare industry in 2009. Rush leads practice coaching programs that utilize QI coaches from local industry assigned to 1-2 practices to support practice transformation. He has developed innovative tools and resources and curriculums to support changes in health care settings.
