1 ARIZONA EARLY INTERVENTION PROGRAM

2  States must report annually to OSEP on the primary early intervention service setting for all children reported in the annual Child Count report  This data are reported on 618 Table 2, Settings  The due date for this report is February 1 each year.

3  Each local early intervention program must provide early intervention service setting data to DES AzEIP for all children with IFSPs ◦ Programs must report setting for each service on the IFSP ◦ Programs must report the primary service setting for each child with an IFSP  Settings data is reported by local programs in the database submitted to DES AzEIP

4  For each service on the IFSP, identify the service setting: ◦ Home- the principal residence of the child’s family or caregivers ◦ Community-based- Setting where children without disabilities typically are found. Includes child care centers and family child care, preschools, early childhood centers, libraries, parks, grocery stores ◦ Other- Settings that are not home or community- based. Includes hospitals, clinics, programs for children with disabilities

5  For each IFSP, identify the primary setting ◦ Primary setting is the setting in which the child receives the most hours of early intervention services ◦ Based on the setting information on the IFSP  The setting data on the IFSP is entered into the database (ACTS, Focus, ECFE)  See the Settings FAQ for more information.
