Informatics Activities to Support the 2009-2010 H1N1 Vaccination Campaign Loretta A. Santilli, M.P.H. NYSIIS Program Manager

Patient Level Information Tracking Individual level data collection will be required for every dose of H1N1 vaccine administered, regardless of the age of the recipient. Data collection tool for external partners New York State Immunization Information System (NYSIIS) New York State Immunization Information System (NYSIIS) Electronic, statewide database (i.e. authorized users have access to entire database) Electronic, statewide database (i.e. authorized users have access to entire database) Widely used by LHDs, pediatric providers, birthing hospitals (2,200+ orgs, 6,000+ users) Widely used by LHDs, pediatric providers, birthing hospitals (2,200+ orgs, 6,000+ users) Limited use (but available to any) adult providers, health care facilities, etc. Limited use (but available to any) adult providers, health care facilities, etc.

Authority to Collect Info As of January 1, 2008, PHL2168 requires reporting immunization information for all persons less than 19 years of age. Information for 19+ can be included with “express written consent of the vaccinee” (not required) Anticipating Governor’s emergency declaration which will suspend this section of law and temporarily allow influenza vaccination information to be recorded in NYSIIS for adults without written consent. Anticipating Governor’s emergency declaration which will suspend this section of law and temporarily allow influenza vaccination information to be recorded in NYSIIS for adults without written consent. Doses administered reporting will be a condition of H1N1 vaccine receipt (Vaccine Provider Agreement).

NYSIIS Enhancements Contractor (EDS) tasked with adding the Countermeasure and Response Administration (CRA) module into NYSIIS (due 9/1/09) Module used by 2 other WIR states, based on CDC DAX specs Module used by 2 other WIR states, based on CDC DAX specs Will allow collection of event/campaign info and priority group designation and creation of aggregate doses administered report per CDC requirements Will allow collection of event/campaign info and priority group designation and creation of aggregate doses administered report per CDC requirements Will need to add new vaccine codes CVX codes just released by CDC CVX codes just released by CDC Will need to modify scheduler to accommodate 1 or 2 dose schedule based on clinical trial results

Provider Recruitment and Registration Three necessary steps: 1.Health Commerce System Account https://hcsteamwork1.health.state.ny.us/pub 2.Vaccine Program Registration 3.NYSIIS Training (Enrollment) https://commerce.health.state.ny.us/hpn/bcdc/immunization/training_info.html Developing a streamlined process to cover all 3 requirements in one electronic workflow.

Additional Consideration: Inventory Tracking From the endpoint perspective (vaccinating organization) NYSIIS has the ability to assist with inventory tracking. State level pre-load of inventory info (obtained from State vaccine/logistics group) Sites would be able to autopick from inventory list, system would decrement as doses administered, low inventory alerts could be set Reduces data entry time, potential data entry errors and increases vaccine accountability
7
Additional Consideration: Adverse Events Tracking NYSIIS has ability to record ‘reactions’ Free text field could collect adverse event info and if VAERS report was filed Potential linkage of NYSIIS immunization history data with health insurer utilization information Monitor diagnoses and id potential ‘signals’ if correlated with vaccine administration Monitor diagnoses and id potential ‘signals’ if correlated with vaccine administration i.e. Guillain-Barre Syndrome, miscarriages, etc. i.e. Guillain-Barre Syndrome, miscarriages, etc.
