The PST recommends that an individual be transferred from a Training Center to a nursing home or congregate setting with five or more individuals; Section IV.D.2.a. RST Referral Process: Training Centers Referrals to CIM occurs when:

15 Page 15 DBHDS Virginia Department of Behavioral Health and Developmental Services b. the PST is having difficulty identifying or locating particular types of supports within: 90 days of discharge plan (2013) 60 days of discharge plan (2014); 45 days of discharge plan (2015); and 30 days of a discharge plan thereafter. Section IV.D.3.a. Takes effect March 2014.
