CASE MANAGEMENT SERVICES FROM INTAKE TO EXIT FROM THE PROGRAM You and your case manager will arrange a meeting Together you will develop a post program plan for your Roadmap to Success and Individual Career Plan You will also create a Program Release plan together You will discuss your last meeting incentive You will discuss your successful completion of the program THIRTEENTH MONTH Your Evaluation and Post Program Plan and Completion of the Program You will have monthly check-ins with your case manager from your sixth week until your thirteenth week They will provide you with ongoing case management services The case manager will re-evaluate and update your case plan You will discuss your outcomes and self- sufficiency You will review your Resource Checklist WEEK SIX-TWELTH MONTH Help Promote your self- sufficiency and self- empowerment through maintenance and tracking your advancement Your Case Managers will Help you Track your Progress with the Roadmap To Success Program Review your Individual Career Plan And Individual Training Plan Help you establish your ”Next Steps” and reevaluate your goals Create a Resource Checklist (3-6-9-12 month) Develop a release plan Discuss and help you with concerns you have about releasing Give you information on medical/dental resources Address any other areas that you need help with that they may be able to help with WEEK FIVE Review Your Career Plan and Training Plan Tracking Your Progress and Adjusting your Plans and Needs for maximum Positive outcome of Program Objectives After you complete the Roadmap to Success class You will create a vision of yourself as a successful and productive part of society Your case manager will help you Evaluate your case management plan Re-evaluate your Individual Career Plan Re-evaluate your Individual Training Plan Develop a budget plan Develop a Roadmap To Success Exit Plan FOUR WEEK Implementation of Case Plan Vision of your Future and Address Your Barriers Mission services will work with you to establish goals It's called the roadmap to success With Your Case Managers Help You will identify your personal goals Your recovery goals Your professional Goals Your case manager will help you Create a resource check list (0-3-6-9-12 month) Create a needs form Create an Individual Career Plan FOUR WEEK Roadmap To Success Intensive Services Assessment and Understanding of Goals Action Plan Job developers work with you for career guidance and job referral Training: Resume writing Interviewing skills, Case Management: Housing Transportation Assistance Health: (Inc., MH) Legal Issues Child Care resources (Head Start, ECEAP) Clothing Assistance WA State Driver’s License Budgeting Plan DVR Resources Goal planning: Goal, Objective, Action plan WEEK ONE Information about client (You) Release of information Form Referral form Do you have any outstanding warrants, in which counties?
