6 ICM and Wraparound… I NTEGRATED C ASE M ANAGEMENT Multi-system collaboration that uses wraparound principles to provide a sustainable INFRASTURCTURE to coordinate services for vulnerable youth/families with complex needs involved in Child Welfare & Juvenile Justice. Along with statewide overarching goals/outcomes, Skagit, Okanogan, Thurston, and Pierce County implementation sites set the practice model and the outcomes. W RAPAROUND Multi-system collaboration that uses the wraparound principles and a specific multi-stage practice model to coordinate services for children and youth (the what and the how have national standards. “High Fidelity” sites are evaluated using a specific tool to ensure that the practice adheres to the principles.

9 The Skagit Model to Date… M ULTI -S YSTEM L EADERSHIP T EAM DISCUSSES A YOUTH & FAMILY WHO MIGHT BENEFIT FROM A MULTI - SYSTEM MEETING. P ARENT P ARTNER CONTACTS FAMILY, DESCRIBES THE PROCESS, ASKS IF THEY WANT TO PARTICIPATE, AND FACILITATES CONNECTION TO THE LOCAL TEAM. T EAM MEETS WITH PARENT AND YOUTH ( IF APPROPRIATE ) TO BETTER UNDERSTAND THEIR NEEDS AND HOW THE COMMUNITY CAN COME TOGETHER TO MEET THE NEEDS. M EETING IS PROFESSIONALLY FACILITATED RESULTING IN ACTION PLAN THAT SUMMARIZES THE NEXT STEPS FOR SYSTEM REPRESENTATIVES AND FAMILY. P ARENT /Y OUTH COMPLETE SATISFACTION SURVEY. A CTION STEPS ARE CARRIED OUT.
