Purpose of the Transition Academy: To provide 18-22 year old young adults with cognitive disabilities the opportunity to receive realistic vocational experiences, while developing skills needed to successfully transition into a more independent life.

Reason for the Transition Academy IDEA mandates the provision of transition services to address the needs of youth with disabilities moving from school to post-school environments. The intent is to improve post school outcomes for youth with disabilities. College and Career Ready Performance Index (CCRPI) Parents are interested in options for their 18-22 year old children

Number of Students this school year: 11 (EHS, RRHS, WHS) Projected number of students for 2015-16: 16 Jobs on Site: Office/Clerical Related Jobs, Media Center, Cafeteria, Coffee Shop, Grocery Store Service Community Job sites: restaurants, assisted living facility, cafeterias, pharmacies, laundry, Salons, Retail Volunteer Sites: Hope Center Baby & More Thrift Shop Cafeteria Sites: In the kitchen: 5 locations Community Events: Picnic in the Park, Elm Street Theater, C.A.T.S. Bus (Outlets), Just People Village Outside Agency participating in IEP’s: Vocational Rehabilitation, Cherokee Day Training Center, Briggs and Associates, and Next Step Ministries Outside Agencies CBI Trips: Cherokee Training Center, Goodwill Career Center- Vocational Rehabilitation Transition Academy WHS

2015-16 Transition Academy Academies: Transition to Work- Provide young adults with cognitive disabilities the opportunity to receive realistic vocational experiences, while developing skills to successfully transition into a work community Transition to Independence- Provide young adults with cognitive disabilities the opportunity to receive realistic vocational experiences, while developing skills to successfully transition into the community Location: CHS: North Zone, CHS/CVHS/SHS WHS: South Zone, WHS/EHS/RRHS Transportation: Students will be transported to and from the Transition Academies, according to North and South Zone, via a shuttle system. Transfer for Curricular Programs- Will we need this? Staff: All staff members have participated in Community Based Instruction (CBI) and Community Based Vocational Instruction (CBVI) training.

Admission Criteria Admission Criteria: both Programs 18-22 years of age, completed all graduation requirements, participate in the GAA and completed all Core Content area units Student has participated in the CCSD CBI/CBVI program in 11 th and 12 th grade. Teacher recommendation for the program and a Transition Services program Qualifying Rubric has been completed by teacher and Transition Coordinator Student has a Community Based Vocational Score determined through data collection of Work Behavior Skills.
